Cancellation:

Full refund will only be granted up to 2 weeks before day of event
(your cancellation must be received by end of day Friday, June 18th, 2013)

Liability Release Form

In consideration of my being accepted by the Amateur Observers Society of NY as a participant of the 2013 AOS
StarFest to be held at the Shady Pines Campground located in Savoy, MA on July 12th through July 14th, 2013:

I, the individual, sending this registration form agree to hold harmless the Amateur Observers Society of NY, their
members, officers, and agents, from any claims, liabilities, losses, demands, causes of action, suits and expenses
(including attorney fees), which may directly or indirectly be connected to the AOS StarFest.

I further agree to indemnify any party indicated above should such party suffer any claims, liabilities, losses, demands, causes of action, suits and expenses (including attorney fees), caused directly or indirectly by my negligent or intentional acts, or failure to act, or if such acts or failures to act are directly or indirectly caused by any person in my family or party / associates while participating in the AOS StarFest.

My submitting this registration form also indicates agreement and acceptance on behalf of other adults in my party and all minor children (under 18 years of age) under my care in attendance.
